Forms in Speybl allow you to efficiently collect important data, get feedback, or manage internal communication. Create any Forms, set their visibility and anonymity, and easily manage employee responses directly in the application.

🔍 What is it for?

Forms are an ideal tool for collecting data, feedback, and conducting internal surveys. They also support anonymous mode. You can use them for HR questionnaires, evaluation surveys, improvement suggestions, and reporting non-standard behavior.

🧾 Create a new Forms

  • Go to the Forms section and click the "+" button

  • Enter a name, description, and other Settings

  • The form can be edited later – including questions, visibility or attachments

🔐 Forms anonymity

  • Anonymous — responses cannot be traced back to a specific user

  • Non-anonymous — each response is associated with a specific employee

  • Optional — respondent decides whether to answer anonymously

Anonymity cannot be changed after creation.

👁️ Visibility and attachment Settings

  • Forms visibility can be adjusted — a hidden form can be made public only at the right moment

  • Attachments — you can allow uploading attachments (photos, documents) as part of the response

🎯 Choice of reply recipient

You can set whether the respondent chooses who to send the form to, or whether it is automatically delivered to everyone with permissions. Permissions: Display or edit.

  • All created Forms are displayed in the overview

  • Clicking on the eye icon will take you to the detail

  • Use the "+" button to add new questions

  • The form can be edited, duplicated, or deactivated

📊 Display answers

You can see individual employee responses in the Forms details — unless the form is anonymous. Responses can be browsed, filtered, and exported.

🚨 Pre-prepared form: Whistleblowing

The app features a pre-built workplace misconduct reporting form, providing employees with a safe and anonymous channel to report inappropriate behavior. The Speybl app protects whistleblowers’ anonymity and promotes transparency in the company culture.
